Reactivation Request

To be eligible to be reactivated with Elite Health Solutions the following must be true:

  • Employee must have worked within the last 2 years.
  • Employee must be in good standing with Elite
  • Employee must be able to turn in current credentials

If it has been more than two years since you have worked then the application process must be started again. Click here to update your application. Once your application is updated, please call the office to set up an interview. 

Please note, Elite has the right to deny any reactivation request for any reason.

  • Current Driver's License 
  • Social Security Card
  • Current TB Skin Test
  • Current CPR Card
  • Nursing License
  • ACLS (optional)
  • Immunization Records
  • Resume
  • Certification
  • Fit Test
  • Nursing Diploma
  • IV Certification  

What Credentials Elite Needs from Nurse Aids, Caregiver, and DSW

  • Current Driver's License 
  • Current TB Skins Test
  • Current CPR Card
  • Social Security Card
  • Immunization Records
  • Resume
  • Certifications

Turning In Credentials

To turn in the credentials you can do one of the following:

  • Bring them into the office 
  • Fax them to 318-213-8484
  • Email them to megan@elitehealthsolution.com 

Once you have turned in all credentials, and your case has been reviewed, you will be contacted by someone in the office.
